Every therapist in New York who has tried to get paneled knows the feeling: a CAQH profile that never seems complete, a 90-day silence from a payer, a rejection for a missing document you were never asked for. Credentialing is not hard. It is tedious, slow and unforgiving of small errors. Here is what it actually costs you to do it yourself, and when it makes sense to hand it off.
What credentialing involves in New York
- A complete, attested CAQH ProView profile, re-attested every 120 days.
- Individual applications to each payer: Aetna, Cigna, UnitedHealthcare/Optum, Empire BlueCross BlueShield, Oscar, and the Medicaid managed care plans (Fidelis, Healthfirst, MetroPlus, etc.) if you want them.
- Contract negotiation, or at least a review of the fee schedule you are offered.
- Follow-up every two to three weeks, because applications stall silently.
- Re-credentialing every two to three years, per payer.
The real cost of doing it yourself
Most therapists spend 15 to 30 hours per panel across applications and follow-up. Four panels is 60 to 120 hours. At a session rate of $150 to $200, that is $9,000 to $24,000 of clinical time, and the opportunity cost of every week you are not yet in-network.
What it costs with MBBCS
Credentialing is part of our hourly service at $30 to $35 per hour. A typical panel takes us four to eight hours of work spread over the payer’s timeline, because we know which documents each New York payer wants and we follow up on a schedule. Four panels usually runs $600 to $1,100 total.
Mistakes we fix most often
Expired CAQH attestation. Practice address that does not match the W-9. Missing PLLC Type 2 NPI. Malpractice certificate with the wrong entity name. Taxonomy code that does not match the license. Each one adds 30 to 60 days.
Quick answers
How much does insurance credentialing cost for a therapist?
Done yourself, 15 to 30 hours per panel of your own time. Through MBBCS, usually 4 to 8 hours per panel at $30 to $35 per hour.
Which insurance panels should a New York therapist join first?
It depends on your area and clients. In NYC, Aetna, Cigna, UnitedHealthcare and Empire BCBS are the most common commercial panels; Medicaid managed care plans matter for many Brooklyn and Bronx practices.
Can I get credentialed before my PLLC is formed?
You can start the individual CAQH profile, but most payers will want the entity, EIN and Type 2 NPI before contracting. Form the entity first.
